What low income programs will help care for my stroke victim father, in my home, during the hours that I work?

One year ago my father had a stroke, and has been in a nursing/rehab facility ever since. He is miserable there, and I want to bring him home to live with me. He has a feeding tube, minimal ability to speak, a catheter, and is paralyzed down his entire right side. I have worked as a nurse's aid and I cared for my ailing mother until she passed, so I know I can care for him while I am home. But I have to work full-time and I cannot afford to pay someone else's wages to come to my home and sit with him ( I am considered low income). Does anyone know of any programs that will help me considering that he is a Pennsylvania resident and I live in Illinois?
